If not, it will be returned to you and will take longer to process. Please make sure all the sections are filled out. Please print and complete the application (three pages), child abuse clearance form (three pages), commitment and confidentiality form (two pages), Consent/Release of Information Authorization Form (two pages) and the Criminal Record check disclosure form (one page).
